Slip and Fall Investigation Checklist
- Call 911 if anyone is hurt.
- Document the location of the accident, a general description of the scene, and date and time of the accident.
- Take photos of any substance, debris, or unsafe condition that caused your fall. For example, any missing or broken tiles, torn carpet, or a liquid.
- Take a video or photograph of the walkway surface where you slipped/tripped. For example, it could be wood, ceramic tile, vinyl tile, terrazzo, marble, concrete, asphalt, grass, brick, gravel, etc.
- Take photos of anything that may depict the length of time the unsafe condition existed.
- Take photographs of the accident site, including the area surrounding where you fell in order to find the source of the unsafe condition that caused your fall which could be a lack of warning signs, mats, or broken equipment, such as a refrigeration unit.
- Take photos of anything that appears to be surveillance, which may have been monitoring the area you fell, or if relevant, the lighting conditions in the area.
- If you tripped as a result of a raised sidewalk, place a ruler against the sidewalk where you tripped and document the amount of displacement.
- Ask to fill out an incident report – and take a picture of the report just in case they decide they do not want to provide you a copy.
- Obtain the names and contact information of any witnesses that may have seen you fall or seen the source that caused your fall.
- Obtain the names of the people persons taking the incident report and the names of store employees that have knowledge of your fall or the condition that caused your fall.
- If you fell due to unsafe stairs, take pictures of the stairs and be sure to include the handrails, or the absence of handrails. If possible measure the risers to document whether or not they are even.
