Slip/Trip and Fall Checklist

  • Call 911 if anyone is hurt.
  • Document location of the Accident, including general description of the scene, and date and time of the accident
  • Take photos of substance, debris, or unsafe condition that caused your fall (this could be for example a liquid, missing or broken tile, torn carpet)
  • Document and/or take photo of the walkway surface that you slipped/tripped on (i.e. Wood, Ceramic tile, Vinyl tile, Terazzo, Marble, Concrete, Asphalt, Grass, Brick, Gravel, etc)
  • Take photos of anything that may depict length of time the unsafe condition existed
  • Take photographs of site of incident (this includes the area surrounding where you fell which may depict lack of warning signs, lack of mats, perhaps a refrigeration unit or other equipment that is broken, or otherwise depict the source of the unsafe condition that caused your fall
  • Take photos of anything appearing to be surveillance that may have depicted the area you fell, or lighting conditions if relevant
  • If you tripped as a result of a raised sidewalk, place a ruler against the area you tripped to document the amount of displacement
  • Ask to fill out an incident report (and take a picture of it in case they decide they do not want to provide you a copy)
  • Obtain names of any witnesses that may have seen you fall or seen the substance that caused your fall.
  • Obtain names of persons taking the incident report and names of store employees that have knowledge of your fall or the condition that caused your fall
  • If you fell due to unsafe stairs, take pictures of the stairs including handrails (or absence of handrails). If possible measure the risers to document whether or not they are even.
  • If you have any pain, see a doctor ASAP
